Harm Reduction Case Manager Role
Under the direct supervision of the Case Management Supervisor, the Harm Reduction Case Manager will play a crucial role in linking clients to care through our Harm Reduction Program.
The primary responsibility of the Case Manager will be to successfully connect clients to community-based programs addressing medical and psychosocial needs of individuals who use substances, particularly treatment for substance use disorder and infections related to injection drug use (IDU), including viral hepatitis, HIV, and bacterial infections such as endocarditis.
The Harm Reduction Case Manager will assist clients requiring multiple linkages to access their desired program. Potential assistance may include help applying for identification, Medicaid/health insurance, or unemployment benefits.
They will link clients to non-judgmental programs that can service active individuals using substances and are culturally competent to meet the needs of this community.
They will ensure newly diagnosed HIV+, Hep C+, or syphilis reactive clients are referred and linked to medical services.
They will provide harm reduction and overdose awareness to clients and various organizations within the community.
The ideal candidate will possess in-depth knowledge of the Los Angeles healthcare system.
Diversity, Equity, and Inclusion are core values at Venice Family Clinic.
